[AMBER-Developers] Print options for Amber manual

In the past, we have used lulu.com to create printed versions of the manuals,
although not very many people have taken advantage of this.

Lulu has discontinued offering spiral binding in the Crown-Quarto size we
have used before. The only real option for spiral binding is now USLetter.
We could still do a perfect binding with the old size, but spiral (coil)
binding is a lot nicer.

So: (a) does anyone know of good alternatives to lulu? I've spent a while
surfing the web, but many other publishers don't have spiral binding either.

(b) do you have opinions about an 8.5x11 manual? Would anyone use it? Note
that the maximum number of pages for spiral binding is 470, so we would have to
split the manual into two pieces, and or just print a "basic" version with
some chapters missing.

(c) Same question as (b), but for a perfect-bound (ordinary paperback binding)
at the old size.
